Palmetto Playhouse LLC Liability Waiver

1. Assumption of Risk and Release of Liability

I understand that participation in activities at Palmetto Playhouse LLC involves inherent risks of injury, including but not limited to slips, falls, collisions, and other risks associated with soft play. By signing this waiver, I voluntarily assume all such risks on behalf of myself and any minor(s) I am responsible for.

I understand and agree that Palmetto Playhouse LLC LLC, its owners, employees, agents, and representatives shall not be held liable*

2. Minor Participation

If I am signing this waiver on behalf of a child, I confirm that I am the child’s legal parent or guardian, or a responsible adult with permission from the child’s legal guardian. I grant permission for the child(ren) listed to use the playroom and equipment.*

3. Physical Condition

I acknowledge that the participating child(ren) is/are in good physical health and do not have any medical conditions that would make participation unsafe or increase the risk of injury.*

4. Safety Compliance

I agree to follow and ensure that all minors in my care follow all posted safety guidelines and staff instructions. I understand that failure to comply may result in injury or removal from the facility without refund.

5. Grip Sock Policy (Required)

To ensure safety and hygiene, grip socks are required for all children and adults entering the play area. Shoes and bare feet are not permitted. I understand and agree that:

Grip socks must have non-slip bottoms.

Socks are available for purchase if needed.

Entry may be denied without refund if this policy is not followed.*

6. Release of Liability

I hereby release, discharge, and hold harmless Palmetto Playhouse LLC from any and all claims, demands, actions, or causes of action arising from or relating to injury, loss, or damage (including death) sustained during participation at this facility, except where prohibited by law.*

7. Indemnification

I agree to indemnify and hold harmless Palmetto Playhouse LLC from any and all claims, actions, liabilities, damages, or losses resulting from our participation, except in cases of gross negligence or willful misconduct.*

8. Supervision Requirement

I understand that children must be supervised at all times by a responsible adult. Drop-offs are not permitted. I accept full responsibility for the monitoring and behavior of the child(ren) in my care.*

9. Lost or Stolen Items

I understand that Palmetto Playhouse LLC is not responsible for lost, misplaced, or stolen personal items.*

10. Illness & Health Policy

I confirm that neither I nor the child(ren) in my care are experiencing symptoms of illness. I acknowledge risk from exposure to illness in public spaces.*

11. Medical Authorization

In the event of a medical emergency, I authorize Palmetto Playhouse LLC staff to seek emergency medical treatment for the child(ren) in my care.*

12. Photo/Video Release (Optional)

I consent to the use of photos or videos of myself or the child(ren) for marketing and social media.

I agree that I am responsible for myself and the child(ren) listed and understand that failure to follow rules may result in removal without refund.*
